(Not) Human Error is an assassination challenge in the Situs Inversus mission in Hokkaido.
To acquire this challenge, Agent 47 must assassinate Erich Soders by performing his surgery himself and botching it.

Walkthrough[]
- First, acquire the Helicopter Pilot outfit. The easiest way to do this is to knock out the guard in the guard room (under the helipad), then knock out the pilot when he comes in.
- Second, acquire the Chief Surgeon outfit. Meet with the chief surgeon on the helipad and lead him down to the guard room.
- With the Chief Surgeon outfit on, go into the robot surgery control room. Use the three options in order of right to left (activate robot arm, inject cardiac syringe, extract blood).
