(Not) Malpractice is an assassination challenge in the Situs Inversus mission in Hokkaido.
To acquire this challenge, Agent 47 must assassinate Erich Soders by having the chief surgeon, Nicholas Laurent kill him himself.

Walkthrough[]
In order to achieve this challenge, the player must first obtain the evidence from Yuki Yamazaki's laptop, available in her suite. This is straightforward if you have the bodyguard uniform. The fastest way to obtain a bodyguard uniform is by going to the security room in the medical wing, near the morgue, where the lone bodyguard who stands watching the monitors is easily pacified. Climbing in from the adjacent suite (Portman's) is possible, but difficult, since Yamazaki's room is occupied by multiple bodyguards, including two on the balcony.
The second prerequisite is the most difficult: obtaining the blue surgical scrubs worn by the operating team. Almost all the surgeons are currently in the operating theater conducting the surgery. The easiest way to obtain the surgeon's disguise is to complete the mission story Makeover, which involves first obtaining the disguise of Jason Portman (the bandaged patient.) Portman starts the game in the public wing but will eventually be summoned to the medical wing as part of his recovery from recent surgery. Once escorted there he will find himself alone in a room with a surgeon, allowing the player to obtain this uniform.
If that opportunity has passed, it is still possible, though tricky, to obtain the surgeon's disguise. There is one surgeon in the physical therapy room, a room guarded by an immobile sentry stationed in the corner who can see through the bodyguard disguise. The surgeon himself can see through both the bodyguard and the doctor disguises. The surgeon cycles between the PT room and a nearby hallway, passing through the large doctor's office. Poisoning his water with emetic does not work, since he will go to a trash can to throw up rather than a bathroom. Obtaining his disguise involves waiting for the surgeon to leave and positioning oneself in the doorway behind the bodyguard's shoulder (in the large exercise room). Using a thrown object to move the bodyguard to the far end of the room, knock him out quickly and stuff him in the cabinet before the surgeon returns. Complicating this process is the presence of a mirror, the two bodyguards at the far end of the exercise room engaged in a push-up competition, and the fact that Jason Portman occasionally wanders through here. It is possible to remove the bodyguards from the room by beating them in their push-up competition. When moving either body to the locker, make sure the two aforementioned bodyguards are not looking in your direction, as the door is glass. With patience and proper timing it is possible to obtain the disguise without killing or being compromised.
Once the surgical scrubs have been obtained, move to the surgical theater through the doors that are down the hall from the helipad entrance. The terminal in the second room can be interacted with. Load the evidence on to the terminal when nobody is looking. Dr. Laurent (recognizable by his red bandanna) will eventually use this terminal and read the files. However, while he will be deeply upset, it will not be enough to persuade him to murder his patient, as the doctor is quite disciplined and professional. He will still cycle between the control room and the helipad, with a detour to the staircase for a phone call that reveals some minor plot information.
The final step involves obtaining the pilot's uniform. The pilot, nicknamed "Nails", has a side hustle as a drug dealer, regularly supplying Dr. Laurent with pills. Nails moves between his pill stash in the security room with the sleeping guard, and the helipad. Knock him out and put him in the security room's cabinet, then wear his uniform, being aware of the roaming handyman who walks the stairs. The sleeping guard should not be a problem. Return to the helipad and wait for Laurent. When he asks for another fix, lead him down to the pill stash, where Laurent will take too many. Now high, Laurent will walk back to the operating theater, raving about revenge, and will eventually murder Soders.
