The Sicilian Mafia (also known as Cosa Nostra) is a crime syndicate that emerged in the mid-nineteenth century in Sicily, Italy. The Mafia's leader was Giuseppe Guillano. The Mafia kidnapped Emilio Vittorio and asked Agent 47 for a ransom of $500,000 dollars.

The Sicilian mob are usually well organised with contacts in high ranking places, there compound in Sicily is well guarded with Guards on nearly ever-door of the compound. As shown in Hitman 2: Silent Assassin they do have minor weak spots in the security chain making infiltration of the compound possible.
