After Colorado we did some digging into Soders' private affairs, and discovered that he has been fast-tracked for critical heart surgery at the hyper-exclusive Gama private hospital in Hokkaido, Japan. Such a display of power has Providence written all over it.
We have booked you into Gama under the usual guise of Tobias Rieper, corporate shark, here for a standard medical checkup. As such, you will need to play it by ear and procure whatever tools you need to complete the mission.
You also need to eliminate Yuki Yamazaki; a Tokyo lawyer who works for Providence. Soders has already given Yamazaki access to our client records and has agreed to provide a full list of active ICA operatives, post operation. This transaction cannot be allowed to happen.
Soders must pay for his treachery and his insidious employers must be taught a lesson. ICA’s sovereignty is at stake. Powerful as Providence may be, we need to draw a line in the sand.

Having discovered that Erich Soders has been secretly colluding with the organization known as Providence, the ICA Board of Directors has sanctioned Agent 47 to eliminate Soders, both to prevent more leaks, and as a show of force against Providence.
Soders, who suffers from the medical condition Situs Inversus (where his internal organs are lethally and erratically arranged), has used his connections with Providence to secure himself a right-sided heart, and a surgery at the high tech GAMA hospital in Hokkaido, Japan. Agent 47 must infiltrate the hospital and eliminate Soders. Additionally, he must eliminate Yuki Yamazaki, a Tokyo lawyer and a member of Providence's messenger corps, The Heralds, who has been sent to retrieve a list of active ICA agents from Soders.
Some time after the mission, Diana is approached by The Constant, Providence's controller. He promises to no longer interfere with the ICA, and requests that Diana help take down the Shadow Client. In return, he promises to exchange information about 47's past...

Dexter has been here so frequently that we, despite Dr. Ito's best conservation and curation efforts, are currently out of viable lungs for him to have transplanted and Mr. Dexter is now on the RED list, awaiting a suitable donor organ. It should also be noted that Mr. Dexter still smokes and multiple staff members have confiscated several packs of cigarettes from his room; despite our best efforts, new packs keep showing up. It is recommended that an extra eye is kept on Mr. Dexter after surgery to ensure this habit is killed. |

One such heart did fortunately find its way to Gama and currently resides in the organ vault. Dr. Nicholas Laurent has been flown to assist KAI with the heart replacement procedure. Mr. Soders is currently being prepared for the procedure through a series of ground-breaking stem cell treatments pioneered (patents pending) by Dr. Laurent. The patient resides in theater and is under constant care from KAI and dependent on life support. Patient has not yet been put under. |

The procedure proved to be quite a challenge given the weak chin and almost-non-existent cheekbones of the patient and his desire to match Kruger 1:1. Previous facial surgery complicated matters considerably due to a lack of cartilage and facial support structure that had been chiselled away over the years. Patient is scheduled for checkup today. |

Mr. Rieper was admitted to Gama for a standard physical checkup. Preliminary results indicate that Mr. Rieper is in peak physical condition. In fact, the tests show absolutely no indication of any of the issues one might expect from a man his age. Melanoma, cholesterol, diabetes, cancer and heart condition checks are all in the green. Physically, Mr. Rieper is like that of a man in his early 20's. KAI was asked to run all test results through her algorithms twice. Note from the supervisor: Would it be possible to keep Mr. Rieper at Gama a little longer to run some additional test? This man is literally one big stem cell. It's almost as if he's been engineered. |

- There are a total of 169 civilians in this mission.
- This mission can be compared to the Hitman 2: Silent Assassin mission Terminal Hospitality; both missions take place in an exclusive hospital, and the assassination target is undergoing a heart surgery; in the case of Terminal Hospitality, the target is Deewana Ji.
- This mission is the second in the series to take place in a hospital (not counting the Asylum or the Pine Cone rehab clinic).
- This is the first mission in the entire franchise that does not require you to assassinate a target directly since Soders will meet his implied death if the heart was destroyed.
- This is similar to Murder at the Bazaar in Hitman 2: Silent Assassin where it is not mandatory to kill the targets to be able to finish the level, though the objective to kill them still remains incomplete.
- Situs inversus is a real condition. Patients with situs inversus who do need a heart transplantation do not actually need a right-sided donor heart; this and the fact that Soders could receive an artificial heart means that it would be unlikely for a man with his resources to be doomed simply because he needed a new heart. Medical facts were adjusted to enhance the game's theme of the wealthy and elite of society exploiting the poor and vulnerable-in Soders' case, by harvesting the organs of an impoverished person in the developing world to prolong this own life.
- The challenge "Jeg Sauna Hende Ikke" is a play on the Danish phrase "Jeg savner hende ikke", which in English means "I don't miss her".
- The Opportunity "Tell-Tale Heart" is a reference to Edgar Allan Poe's short fiction "The Tell-Tale Heart".
- The window to the operating theater is bulletproof, despite similar windows not stopping bullets.
- 47 hasn't improved much when it comes to dancing from his days in Hitman: Blood Money. This is evident when completing the "Techman" challenge. However, this doesn't apply if he is in the Ninja suit.
- Leaving an explosive near the windsock (wind direction flag) that is near the helipad, followed by acquiring the disguise of the chief surgeon, and then finally interacting with the operating console will start a game of Hitmine, a parody of Minesweeper. Winning the game will cause the chip-tune version of Ave Maria to play, then an explosive rubber duck will fall on Soders and another one will fall near him, followed by a large explosion in the operating theater, killing Soders and the nearby nurse.
- KAI was inspired by HAL 9000 from the movie 2001: A Space Odyssey, and will also sing part of The Blue Danube Waltz when she is sabotaged.
- If 47 approaches the room where a guard is doing pushups while being disguised as a bodyguard himself, he can do pushups as well, outcompeting the guard with ease. If he then sabotages KAI and returns back to the push-up room, a trapped AI, Ayakashi, will appear. He will claim that he is better than KAI, and that KAI is a Nazi, and ask 47 to free him. Ayakashi can be freed by escorting him to the snowmobile exit. Returning to KAI after freeing Ayakashi will reveal the relationship between KAI and Ayakashi.
- If 47 Approaches Soders in the default suit he will be able to reveal his identity to Soders, causing him to have a heart attack and die.
- There are 10 Mini Ninjas statues scattered on the map. Destroying them will cause a volcano to erupt.
- The sauna presents a unique opportunity in the game in that it is a damaging environment that is not poisonous in nature. Anybody knocked out inside the overheated sauna will eventually die. These are considered accidental deaths, but when creating contracts in Contracts Mode the cause of death is not specified for the targets who have been killed this way, and thus the player cannot set a kill condition for them.
- There is an occasional glitch when starting in the morgue disguised as a corpse where 47's body sits upright on the gurney but does not leave the hiding spot, which understandably alarms the morgue doctors. This can occur if the player manipulates the controls as the level is starting but before the player can take control of 47.
- There was originally meant to be a third Elusive Target in Hokkaido, entitled The Rogue. However, due to Square Enix severing ties with IO Interactive in 2017 and the funding subsequently lost, it was re-purposed to be a fully-fledged mission in the Patient Zero bonus campaign in the GOTY edition of HITMAN™, entitled Patient Zero; however, it was altered.
