Following the events of Mendoza, 47 awakens to find himself in a medical lab, installed on a military train heading through the Carpathian Mountains. Breaking free from the car, he makes his way to the front of the train to complete one final mission - eliminate Arthur Edwards once and for all.

A technical report on Ether's new memory-repressing neural-inhibitor serum. The formula is reverse-engineered and enhanced by a much older prototype created at the "Institute For Human Betterment" in Romania in the 1980s. An antidote produced at Ether's R&D site in Johannesburg was stolen by Lucas Grey during his assault on the facility in order to unlock 47's hidden memories. In its current version, the serum is a blunt instrument, completely erasing both long and short-term memory banks. But the research team is confident that in just a few cycles, they will be able to target individual memories with massive commercial potential. |

A research report by Ether's Acquisitions Manager on the history of the the train. Nicknamed "The Mortar" after the Baba Yaga's favorite means of transportation, the train was originally a KGB black site commissioned by legendary Sixth Column spymaster Janus. Over the years, it was used as a top secret data archive and mobile prisoner interrogation facility. The train runs on a perpetual loop from Zagreb in the West to Itkursk in the East, cutting across vast uninhabited stretches of Russia, Kazakhstan, Romania, and Mongolia, using a system of decommissioned rail lines. After the fall of the Soviet Union, the train was stationed near Volgograd until it was purchased by the Ether Corporation as a sample backup storage facility. Following the bombing of Ether's R&D site in Johannesburg, they began refitting the train as a mobile field lab. |

There is currently one escalation contract available in the Carpathian Mountains, only available by purchasing DLC:

Target Lockdown[]
When threatened, Edwards will simply stand still, waiting for 47 to kill him. If non-lethally shot he will be knocked unconscious, and cannot be revived because no other NPCs are on the map once 47 confronts him. This makes him one of 3 targets in the World of Assassination trilogy to not have a Target Lockdown, along with Noel Crest and Patrick Morgan.
Trivia[]
- Including the secret ending (see below), there are two alternatives to eliminating Arthur Edwards, making this one of the only missions in the series that gives the player the option of either killing everyone on the map without penalty or, alternatively, nobody at all.
- A hidden outcome can occur if the player waits for 47 seconds after picking up the syringe: Edwards will again urge 47 to inject himself and serve Providence. The player can then choose to comply, injecting the serum into 47, after which Edwards will thank him. Agent 47 then loses consciousness and will eventually wake up in a padded cell, dressed in his iconic suit, on a bed similar to the one in the beginning of this mission. The player will hear Edwards, over an intercom, speaking the very first lines that Ort-Meyer said to 47 in the first game of the Hitman series. Doing so will yield the A New Father Challenge and Count Down From 47 hidden Achievement/Trophy.
- This is very similar to one of the possible endings in a previous Hitman game, the final mission “Meet Your Brother” of Hitman: Codename 47, where the player can fail the mission after being subdued by 47's creator Ort-Meyer, ultimately waking up in the cell where the game began. In this case, it is implied that Arthur Edwards and his Providence operatives now control the Asylum instead of Dr. Ort-Meyer.
- In the Berlin escalation The Satu Mare Delirum, 47 will begin in the same condition: lying unconscious on a gurney and locked in a small room. This is similar to his escape from the Asylum in Training.
- Oddly, even if 47 killed Edwards, he can still inject himself with the serum after waiting the appropriate interlude; and Edwards' voice will still be heard thanking him and later speaking to him over an intercom.
- In The Wrath Termination escalation, 47's physical body is seen lying on an iron morgue bed, exactly like the start of this mission. This indicates the Wrath Termination hallucination takes place moments before 47 wakes up in Carpathian Mountains to confront Arthur Edwards. As it is the final episode in the Deadly Sins series and has a strong focus on open combat, unlike most levels in the game, it presages the similarly combat-heavy finale to the WOA Trilogy.
- According to dialogue from the Providence guards, the train is on a loop from Zagreb, Croatia to Irkutsk, Russia.
- The "I'm the Conductor Now" challenge appears to be a reference to the "I'm the Captain Now" meme.
- Other than the training tests in the ICA Facility, this is the only mission in the World of Assassination trilogy that presents no option to smuggle in any weapons or equipment whatsoever, either to drop-off points or carried on the player's person.
- The May 2023 Patch Preview blog promised that Patch 3.150.1 would remove the disguise restriction for the Laboratory (and presumably Outdoors) Starting Locations. However, this was later revealed to be inaccurate and based on an unconfirmed fix. There has been no mention of implementing this since.
- This mission is currently not available in Contracts Creator.
- The mission's unusual layout (extremely narrow, in rapidly moving surroundings) results in a few physics bugs.
- Eliminating guards by pulling them out of a window can result in the body becoming stuck in the window where it flutters indefinitely in mid-fall.
- Distractions in the final segment of the train containing the office workers can draw the attention of characters on the opposite side of the train (if they have not been pacified or killed), who will then clip through walls and obstacles to reach the area. If not disabled, they will remain in this segment and retain their ghost-like characteristics, continuing to pass through walls or other objects.
- After Shadows in the Water was added, both Noel Crest and Sinhi Venthan appear with the the rest of the targets in "The Reflection".
- On the surface, Diana's actions appear to be a betrayal of 47, but were in reality a complex ruse to place him close to an enemy. This is very similar to the final mission of Hitman: Blood Money, Requiem, in which she plays a similar trick on an enemy organization, pretending to defect to the opposition and appearing to disable 47 while placing him within striking distance of an otherwise unreachable target. Both plots also involve a special serum.
- This is the only mission in the trilogy that permits the player to eliminate anybody on the map without penalty. In fact, the player will gain points for eliminating anybody, even the non-combatant civilians, as all of them are part of Providence and their elimination will further the destruction of that organization, which is by this point Agent 47's only goal. Nevertheless, the player can also choose to eliminate only Arthur Edwards and spare all others, if desired. Also, killing anyone but The Constant will void Silent Assassin rating, as they are still non-targets even if their deaths are not penalized from the score.[1]
- If Arthur Edwards is wounded with a gunshot to an extremity, he will be knocked unconscious, unlike most other NPCs in the game, who will recoil and then react.
- In a similar fashion to the relationship between End of an Era and Slaying a Dragon, Romania was the location where the series began and 47 was created, in the Asylum and Training, and is now the location where the current era of the series ends.
- The map itself, while never confirmed, could be a nod to the Taeyangho Armored Train, both have similar routes through cold northern regions, both are heavily armored, have weapon stores, armed guards, living and entertainment quarters and both contain a figurehead at the helm of national or political importance. The Constant (Arthur Edwards) could be a stand in for the leader of North Korea, Kim Jong Un.
- Its use in the game may also be a nod to the 1995 James Bond movie GoldenEye. There, main villain Alec Trevelyan, who also goes by the name Janus, likewise uses a refurbished and modified Soviet armoured train for travel and as a mobile base of operations. Alec Trevelyan was played by British actor Sean Bean, who plays Elusive Target Mark Faba.
- The 11 Providence commandos and the Providence Commando Leader (Dimitri Belikof) encountered in this level are the only enemies in the entire World of Assassination trilogy to have greater-than-normal durability, due to their heavy armor. They have about 3.5 times the durability of a normal NPC (dying after 17 close-range shots from the HWK21 compared to 5 shots for normal NPCs), and their helmets also allow them to survive multiple headshots. The only other NPC in the entire trilogy with enhanced durability is Dino Bosco, who also wears an armored suit; Bosco takes 20 shots from the HWK21 to kill.
- Note that this does not count the obviously fantastic and non-canonical armored knights that can spawn in the Ark Society if the appropriate Easter Egg is activated.
- NPC Conversation describes the Providence Commandos as being the same CICADA unit that took down Lucas Grey. This implies that CICADA is now completely subordinate to Providence, possibly having been absorbed into the organization.
